Insurance Coverage for Non-Members
Plan 2 Accident Insurance has been designed for:
Non-member Accident Insurance – It covers non members as participants regardless of the length of the activity/event.
Members- During activities/events lasting more than two nights (three nights when one of the nights is an official federal holidays).
Fill out the Plan 2 Enrollment form. Be sure to indicate the following:
Leader name or name of person filling out the form.
Event name/description and location.
Beginning and ending dates — coverage is counted by calendar days including travel time (i.e. - An overnight campout is considered 2 days — the day traveling to the site and the day coming home.)
Compute the cost of coverage ($.11 per person per day). The minimum cost is $5.00. (This amount will cover 45 persons for a one day event.) You may schedule up to five events on one form.
Send the application and troop check made out to Girl Scouts of River Bluffs Council to:

Council approval is required before application is sent to the insurance company. DO NOT SEND YOUR APPLICATION DIRECTLY TO MUTUAL OF OMAHA. Please submit at least one week prior to the event. If this is not possible, please call Barb Boyer at 800-345-6858, extension 141.
